How removal costs mole skin?

My daughter has been begging me for years to get his mole removed (small, slightly elevated. Especially under the nose and upper lip.) Shes 13 and I finally accepted. How much would it cost to get a small mole raised slightly off? Is she old enough? I had some moles removed before and it never hurt. Bites anesthesia Local / clip slightly (like most shots), but not unbearable or the point of tears, and only for one second.
